The team at MyOTCoach requires a Consent Form to be filled in prior to providing services. We also encourage you to fill in the Parent Questionnaire with as much detail as possible for your child so that we may provide the best strategies to suit your needs. Questionnaires for adult only supports will be provided to you by our Occupational Therapist. Please email the completed forms with any questions you may have to:

Fee Schedule
Fees are charged per hour
The rate per hour is $140.00. Services can be direct or indirect and includes report writing, screening and assessment, preparation and administrative tasks. Sessions are offered in-person, at Park Integrative Health, 970 3890 Sherwood Drive, Sherwood Park, AB, T8H 0Z9. Online sessions can be arranged to assist clients who request these due to extraordinary circumstances.
